Psychiatric Collaboration for Clinicians & Practitioners

Julie Megler • Psychiatric Nurse Practitioner

I work with therapists, somatic practitioners, and psychedelic-assisted therapy guides seeking a psychiatric collaborator invested in the quality and continuity of their clients' care.

My approach is evidence-based, integrative, and built around close communication with referring providers. Collaboration is central to my practice.

Clinical Approach

Minimum effective dose. I prescribe conservatively and with clear clinical rationale. Clients leave their first appointment with a thorough understanding of any recommendations made and a defined follow-up plan.

Ongoing communication. I remain available for clinical consultation on shared clients. I am reachable for questions that arise between appointments.

Psychotherapy as a condition of care. For the majority of clients I work with, active engagement in psychotherapy is a clinical requirement. My role is to support the therapeutic work.

Integrative and evidence-based. I prescribe from an evidence-based foundation while remaining knowledgeable about and open to integrative modalities. I will not dismiss or pathologize the approaches you are using with your clients.

Who I Work With

Referrals originate primarily from licensed therapists, LCSWs, psychologists, and somatic practitioners, seeking a psychiatric partner who prescribes carefully and respects the therapeutic relationship already in place.

A secondary referral source consists of psychedelic-assisted therapy guides and integration specialists who require a collaborator with substantive clinical experience in that space, including the ability to assess medical safety and contraindications.

Ways We Can Work Together

Ketamine-Assisted Therapy Collaboration

I prescribe sublingual ketamine for clients engaged in ketamine-assisted therapy with a referring practitioner. To qualify for KAP collaboration, practitioners must hold recognized training in ketamine-assisted therapy, maintain regular therapeutic contact with the client, and be available for quarterly consultation with the prescriber.

Ongoing consultation is a clinical requirement to ensure ethical, informed, and well-supported care throughout the KAP process. I will work with you to coordinate the prescription pathway, preparation, and continuity of care so that the clinical work is well integrated.

I prescribe sublingual ketamine with direct clinician referral.

Psychedelic-Assisted Therapy Practitioner Medical Consultation

Guides and facilitators working with clients in psychedelic-assisted therapy settings are welcome to request medical safety consultation. I review psychiatric and medical history, assess contraindications, and provide clinical recommendations relevant to the planned experience.

This can involve ketamine, plant medicine, or other modalities. An initial 50-minute consultation is required before case review. Once we’ve had a 50-minute intake consultation, you can schedule as needed consults in 15 minute increments. Please come prepared with your client's intake documentation.

Professional and Business Consultation

Available for clinicians navigating private practice development, clinical transitions, or strategic questions about practice structure. With over a decade in private practice, including building, operating, and selling an integrated mental health & ketamine clinic, I offer informed, direct consultation on the business and professional dimensions of clinical work.

Psychedelic-Assisted Therapy

I have been teaching in psychedelic-assisted therapy training programs and academic settings for over a decade, including guest faculty positions at Psychedelic Science, the California Institute of Integral Studies Center for Psychedelic Therapies and Research, and the UC Berkeley Center for the Science of Psychedelics.

I co-authored a chapter in The Therapeutic Use of Ayahuasca and have been prescribing and supervising ketamine-assisted therapy clinically since 2017.

I received formal training through the KRIYA Institute and MAPS, and my background includes direct experience with Southern Traditions, plant dietas, and a range of psychedelic modalities across guided, ceremonial, peer-to-peer, and group settings.

I understand the clinical, ethical, and legal complexity of this work. Providers referring clients who are engaged in or preparing for psychedelic-assisted therapy can expect informed and experienced psychiatric collaboration.

I do not offer or encourage services for federally illegal expanded states.

Beginning the Consultation Process

Most referrals begin with a brief discovery call to confirm clinical fit and discuss the client's presentation before they book. From there, clients schedule a full Practitioner Consult Intake directly.

With appropriate consent in place, I provide referring providers with relevant clinical updates and remain available for ongoing consultation.

Clients leave the first appointment with a clear clinical formulation, documented treatment recommendations, and a defined follow-up plan.

A discovery call is the most efficient starting point as it provides an opportunity to discuss the clinical needs of your client and determine whether Holistic Roots is the right fit.
